Scheduling Your First Appointment

When you call to schedule your first appointment, we’ll ask you for:

  • Your name, address, phone numbers (home, cell, and work), and social security number.
  • Your insurance information (please have your card available).
  • Information about your knee including how, where, and when the injury occurred or symptoms began.

Prompt appointments

We can usually schedule your first appointment within 24 hours of when you call us, Mondays through Fridays.

If the orthopedic surgeon you want to see is out of the office or there is a specific health condition to be considered, the appointment may be delayed.

Motor vehicle accident claims

We can’t schedule appointments for patients with injuries due to motor vehicle accidents as we don’t bill any third-party payers (e.g. auto insurance) for reimbursement.

Private healthcare insurance will not accept liability for claims related to vehicular accident injuries.

What to Bring to Your First Appointment

Insurance Card(s) and ID

Ensure quick and accurate service during your first appointment by bringing:

  • Your insurance card(s)
  • Photo identification.

You will need to pay any out-of-pocket expenses at the time of your visit. Out-of-pocket expenses include co-pay, co-insurance, deductibles, durable medical equipment, etc. We accept checks, cash, Visa, MasterCard, and Discover.

Other Documents

Also bring:

  • Any medical records related to previous knee injuries and/or surgeries.
  • X-rays or MRIs.

If you didn’t submit your New Patient Registration Form online, you can:

  • Print it out and bring the completed paper copy to us.
  • Fill out the form when you come to see us.

What to Wear

  • Sneakers (or other comfortable shoes)
  • Shorts, so our orthopedic surgeon can examine your knee (we can provide shorts if necessary) .

What to Expect at Your First Appointment

Please arrive 15 minutes before your scheduled appointment.

If you didn’t complete your New Patient Registration Form online, please arrive 30 minutes before your appointment time.

Your first appointment could last for 2–3 hours.

What Happens During the Appointment

When you check in, we’ll you ask you to provide and/or verify your information. Please make any corrections regarding your personal and insurance information at this time. If you have not pre-registered, you will be asked to fill out your registration forms. We recommend that you complete the registration forms online prior to your appointment.

After registration, you’ll go to an exam room. We’ll ask about your medical history and your knee injury or condition. If you haven’t had recent X-rays, we’ll take them in our office.

One of our orthopedic surgeons will examine your knee and talk to you about treatment options.

If you need physical therapy, your doctor and a physical therapist or athletic trainer will begin developing your personal physical therapy program.

If you need surgery, we’ll talk to you about this and help you understand what will happen before, during, and after surgery. You need to have the person who will help you after surgery (your caregiver) with you for this discussion. Your caregiver can be anyone you choose who can stay with you for the time required by your procedure and post-operative care. If you don’t have a caregiver with you at your first appointment, we’ll schedule another appointment for you to come back with him/her.

We may schedule your procedure during your appointment if you’re ready.

If you need another appointment, this will be scheduled as you check out.

New Patient Registration Form

Please complete the New Patient Registration Form. You can do this online before you come in for your first appointment.

If you prefer, you can complete your New Patient Registration Form during your first visit.

How to complete the New Patient Registration Form

Please complete the entire form. We need accurate information about your medical history and symptoms to make decisions about the best treatment for you.

After you complete the New Patient Registration Form, click “submit.”

If you don’t click “submit,” or if you close the window before completing the form, your information won’t be saved. You’ll be able to print your form after you submit it to our office.

Shelbourne Knee Center and Community Health Network

Shelbourne Knee Center is part of Community Health Network, a non-profit health system throughout Central Indiana. Community Health Network offers healthcare services through more than 200 sites, including hospitals, health pavilions, and doctor’s offices, as well as in workplaces, schools, and homes.

Indianapolis-based Community Hospital East is one of Community Health Network’s hospitals. Shelbourne Knee Center is part of Community Hospital East.

Billing and Insurance

We are a hospital-based office run through Community Health. Your office visit charge will be divided into:

  • “Facility” charge, billed from Community Health Network (outpatient services).
  • “Professional” charge from Shelbourne Knee Center (with the physicians’ business name).

Other charges will be billed from Community Health as hospital-based services. These charges include:

  • X-ray.
  • Physical therapy.
  • Injections.
  • Durable medical equipment (e.g., canes).
